Commandant (CG-1331) approved a policy change to have Boatswain's Mates submit their CG-5113, District Review Board Evaluation and Reporting Form, to be included in their EI-PDR. This change is slated to be included in the next change to COMDTINST M1080.10 (series), Military Personnel Data Records (PDR) System. However, per e-mail between CG-1331, PSC-BOPS-MR and the BM RFMC, the change is effective now.
Effective immediately, SPOs will forward CG-5113 forms for inclusion in the member's EI-PDR. Submit forms by email to "ARL-PF-CGPSC-PSD-MR_DOCS". Use "URGENT BOARD ENTRY" in the subject line of the e-mail. This will help to ensure the forms are available in the member's EI-PDR for any upcoming retention/screening boards or waiver requests. Detailed instructions on scanning and e-mailing documents to BOPS-MR are in Part X, chapter 4 of the Coast Guard Servicing Personnel Office Manual.
EI-PDR Submissions:
When sending documents for the EI-PDR, SPOs should be aware that:
- Only authorized documents should be sent to Military Records.
- All documents must contain the member's EMPLID.
- Scanned file name must include the member's EMPLID (1234567.pdf).
- Scanned PDF must be authenticated, legible and complete copies of originals.
- File size must be minimized using the guide in Part X, Chapter 4 of the SPO Manual.
- Files shall not be encrypted if e-mailing from uscg.mil or dhs.gov email address.
Common Submission Errors:
These common errors add to the backlog and result in additional processing time for SPOs and MR staff:
- Documents scanned in color (not black and white).
- Documents sent in a foreign language and not translated.
- Documents not marked with the member's EMPLID.
- Documents labeled with another member's EMPLID/SSN.
- Duplicate Submissions.
- Unauthorized Documents.
- Poorly Scanned Images.
- Files with multiple member documents.
- Incomplete Documents (no signatures or missing pages).
- Medical Documents sent in error.
